Feel out the online fraud reporting form. Answer all questions or inquiries to the best of your knowledge and enter all personal information required. Submit the form.
Print the form out and mail it to Social Security Fraud Hotline, P.O. Box 17768, Baltimore, Maryland 21235.
Fax the form to 410-597-0118.
Go to one of the local Social Security offices in Indiana to turn in the form (See Resources).
