Visit the Social Security administration website at ssa.gov.

Complete and submit the change of address form.

You will receive a confirmation letter by mail at your new address.

Telephone

Answer the operator's questions to complete the change of address.

You will receive confirmation by mail at your new address.

Proof of identity may include a state driver's license or state ID card, passport, military ID or school or employee ID with a photo.
